Use of Rapid Rooter Plugs

I have a few questions about rapid rooter plugs.

I generally start seeds using the wet paper towel method. At what point 
should I move the seedling to the rapid rooter plug?

I find that the tap root often breaks when trying to insert the seedling 
into the rapid rooter plug. Is there a proper way to insert the seedling?

I find that although I put the tap root fully into the rapid rooter plug 
  within a few days it has grown out the top of the hole.

Does the rapid rooter plug need to be pushed to the bottom of the cell?
